Abstract: Kratom (Mitragyna speciosa) is a Southeast Asian plant traditionally used for pain, mood disorders, and opioid withdrawal. Its leaves, consumed as tea, capsules, or chewed, contain psychoactive alkaloids mitragynine and 7-hydroxymitragynine which act on opioid and other neurotransmitter receptors. Due to oral intake, liver exposure is high, and metabolism occurs via Phase I/II pathways. Reported liver injury includes cholestasis and mixed patterns. Last 20 years the uses of kratom is expanding in United States (US) and now estimated 1.9 million (0.7%) US citizen use kratom. Kratom is banned in several U.S. states due to safety concerns.
